The best cleanser I have found (I have sled dogs, but they are house dogs I use for my service animals) is a product call Kids and Pets. This can be used as a spot cleaner or put into a carpet cleaning machine, laundry, furniture if the fabric is not type WS or S, on decking and many other cleanable items. Isopropyl Alcohol is another good spot cleaner because it evaverates, taking the other fluids out, but remember never to rub or use pressure on liquids. That will force it further into the carpet instead of pulling it out.

I can't answer for the others...but since we are on this subject, my patio is flagstone and the smell seems to just seeped into the stone. Winter is coming to AZ and I would love to be out back enjoying the weather WITHOUT the horrible smell. =)
Hmmm...this is a quandary. I can't tell you I'm an expert on smell removal, especially dog urine. Let me get Eric in here to help. He might have an answer for you.
Alright troops...I'm gonna try to throw an opinion down for everything i can. I'm not trying to say that this is the only way to do it. I'm just saying that this is what I've tried with successful outcomes.
Cleanser for floors:
If you're going to purchase a cleanser, find Simple Solution. Any pet shop large or small will carry it. It's a very good nice smelling enzymatic cleaner that will make your floor giggle. You can get it in a spray bottle for spot cleaning or you can get it in a big jug to be used in your carpet cleaner. If for some silly reason you can't find Simple Solution, Nature's Miracle works well too.
If you don't want to purchase anything and you just want to do some spot cleaning on your carpet you will want to mix up a 3:1 solution of warm water to white vinegar. Drizzle it on the spot, blot up what you can and then sprinkle some baking soda over it. Once it is completely dry (over nightish), vacuum it all up.
If you're cleaning hardwood or linoleum I would recommend just throwing warm water and white vinegar in your mop bucket and going to town.
For the patio:
The first thing I would try would be a big bucket of 3:1 warm water to white vinegar. Vinegar is really cheap. Just pick up a few gallon jugs, mix it with your water and slowly poor it over the rock with a focus on the cracks and grout/cement between the flagstones. Do it a couple times and let it soak. Yes, it will smell like vinegar for awhile, but it will fade and it's better than dog pee. Vinegar is a great solvent and a good natural antibacterial agent.
Another few things that work really well which I learned from a friend who ran a non-profit rescue is baking soda, just mix to a paste and scrub in, and follow up with a 50/50 mix of lemon juice and water. You do need to be a bit careful with the vinegar as it can lighten colors.
